  • Minimum enrolled varies by product:
    • Dental: 5 employees
    • Vision: 10 employees
    • Voluntary Life and Disability: 20% participation
    • Accident/Hospital/Critical Illness: 15% participation across all three lines
  • Bundling Discounts: Anthem offers multiple bunding discounts off Medical – up to a 4% max:
    • .5% for adding Vision (50% net eligible participation required)
    • 1.5% for adding Dental (50% net eligible participation required)
    • .25% for adding employer-sponsored Basic Life (minimum of $25K)
    • .5% for adding employer-sponsored Short-Term Disability (STD)
    • .5% for adding employer-sponsored Long-Term Disability (LTD)
    • .25% for adding Accident Insurance (15% participation required)
    • .25% for adding Critical Illness (15% participation required)
    • .25% for adding Hospital Insurance (15% participation required)
